All applicants must electronically submit copies of the following documents along with the completed applicant packet. Use the checklist to confirm you have provided the necessary documents.

The following documents are required for certified:

  • Professional Photo
  • Acknowledge all Waivers and Agreements
  • LE Employment History Questionnaire
  • Sworn General Application
  • Birth Certificate or US Passport
  • Driver's License
  • Social Security Card
  • Marriage Certificate and Dissolution of Marriage (If applicable)
  • DD214/Letter of Good Standing (If applicable)
  • HS Diploma/College Diploma/College Transcripts
  • Official HS and College Transcripts (Requested)
  • Military Records Request/Form 180 (If applicable)
  • Background Investigator Waiver/Form 585
  • Out of State year driving history (If applicable)
  • EOT curriculum and In Service Training records (If applicable)
  • ICS certificates #100 and #700IA and HR Files from all Law Enforcement Agencies (Requested)
  • Bankruptcy Discharge documents (If applicable)
  • Police Reports and Court dispositions for all arrests
  • 4 current Neighborhood References

  • 5 Out of State year driving history (If applicable)

  • Police Reports and Court dispositions for all arrests
  • 4 current Neighborhood References
